Northwest Youth Conference Minor Waiver/Release

RELEASE OF LIABILITY FOR MINOR PARTICIPANTS

READ BEFORE AGREEING TO TERMS

In consideration of my child/ward being allowed to participate in any way in the Cheboygan Youth Tackle Football League, also known as Cheboygan Junior Chiefs, related events and activities, the undersigned acknowledges, appreciates and agrees that:

  1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
  2. I FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child/ward’s participation; and,
  3. I willingly agree to comply with the program’s stated and customary terms and conditions for participation.  If I observe any unusual significant concern in my child/ward’s readiness for participation and/or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately; and,
  4. I for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Cheboygan Youth Tackle Football League, also known as Cheboygan Junior Chiefs, it’s directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasee”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
  5. I, for myself, my spouse, my child;s/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child’s/warn involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

                                                        CJC WAIVER  The undersigned hereby agrees to defend, indemnify and hold harmless Cheboygan  Youth Football, also known as Cheboygan  Junior Chiefs or CJC, and it’s officers,                      employees, and agents from and against any and all loss, liability charges and expenses  (including attorney fees) which may arise by reason of participation in any program.  As a parent/guardian of a participating athlete, I understand that football and cheer are contact sports and there are dangers inherent in and the potential for injury and  death involved during playing, practicing and participating.  As a parent/guardian of a participating athlete, I am fully aware and understand that my athlete’s participation in athletics carries with it this risk. As parent/guardian, I hereby consent to emergency treatment of myminor child as a result of accident or injury. I agree to carefully inspect and satisfy myself that the facilities provided are reasonably safe for their intended use. Once having conducted the inspection,  I agree to expressly assume the risk of participating at the premises. Furthermore, I understand Cheboygan Youth Football retains the right to use and reproduce photos, film,videotape, sound clips or otherwise taken during activities. Cheboygan Youth Football also retains the right to reproduce the imageand/or voice of any person who participates in any program, clinic, class or special event and use the same for any purpose withoutany payment. My or my child’s participation in any event or program constitutes my permission to use said likeness.

                                                  Parent’s Code of Conduct   I will be responsible for my behavior as well as for the behavior of those who attend the game/practices as my guest.  I will constantly supervise any non­athlete child in attendance with me at a practice or game and will not allow any object to be brought to any game/practice that could be used to harm others.  I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice or other youth sports events.  I will place the emotional and physical well­being of my child ahead of a personal desire to win.  I will be in control of my emotions.  I will not use profanity.  I will remain in the spectator area during games/practices.  I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all.  I will not advise the coach on how to do their job. If I have any concerns I will wait at least 24 hours after the incident and then speak privately with the coach. If I am still concerned I will then contact the appropriate team representative to seek resolution.  I will demand a sports environment for my child that is free of drugs, tobacco (including smoking), and alcohol and will refrain from their use at all youth sports events or attend a  game/practice under the influence of such.  I will not bring animals other than service animals to any game or practice.  I will remember that the game is for our youth – NOT ADULTS – and that participation in youth sports is a  privilege, not a right.  I will show respect for all participants at all times.  I will do my very best to make youth sports fun for my child.  I will not instruct him to harm any other participants.  I will not coach my child during the game/practice.  I will ask my child to treat other players, coaches, fans, and officials with respect regardless of race, sex,  creed or ability.  I will not make insulting comments to players, parents, officials, or coaches of either team. I promise to help my child enjoy the youth sports experience by doing whatever I can, such as being a  respectable fan, assisting with coaching, or providing transportation.  I will require that my child’s coach upholds the Coaches Code of Ethics.  I will thank coaches, officials, and other volunteers who have conducted the event.    The Cheboygan Youth Tackle Football League, also known as Cheboygan Junior Chiefs, retains the right to remove any individual not adhering to this Code of Conduct.    I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parent’s Code of Ethics Pledge.     I have read the above Code of Conduct and agree to uphold this agreement
